2010-2011 PTSO membership form
The high school PTSO is excited to begin our second year of establishment. We are looking forward to being a strong presence in the high school which will better serve the students, faculty and parents. The objectives are listed below and we hope you will join. Our membership is $10. The membership includes unlimited use of the PTSO discount card good at local restaurants, shops in Dawsonville & free Chick Fila Sandwich coupon.
Objectives:
1. Parent involvement.
2. Better communication between the school, teachers and parents.
3. Purchasing items for the classrooms that could not be budgeted.
4. Purchasing sports equipment for smaller budgeted sports or clubs.
5. Helping supplement educational field trips.
6. Future: Offer yearly college scholarships.

Registration Information
The following items MUST be presented when registering a new student for DCHS:
*Birth Certificate
*Social Security Card (not just the number!)
*GA immunization form #3231
*Proof
of residency (light bill, water bill, cable bill, lease or tax notice)
Telephone and/or cell phone bills will NOT be accepted.
*Withdrawal form from previous school attended
*Legal custody documentation (if applicable)
